A slot is claimed in a single conditional write, so the second of two simultaneous bookings is told the time has gone instead of both being confirmed. This is the failure schools remember, and it is designed out rather than warned about.
After a meeting the state is not 'done' or 'not done' — it is whether a note is still to be written, or an action still to be taken. Leadership sees the outstanding column, not a wall of green ticks.
